Lighten Up by Dr. Audrey Farahmand

November 2013Almost a year after the FDA approved the weight loss drug, Belviq, the drug has become available for physicians to prescribe. Within days of the new drug becoming available, the AMA announced its position that obesity is a disease. This designation, although transparent, would help physicians in being reimbursed for treatment of the condition afflicting the majority of Americans.

The announcement sparked numerous articles from physicians criticizing the AMA for their decision. Opponents of this designation fear this announcement will lead to dependency on pharmacotherapy to manage it rather than lifestyle changes. They also feel that the core issues like making healthy whole foods more affordable and available to poorer populations will be ignored. These opponents would point out that obesity does not necessarily need to be labeled
a disease in order for physicians to be reimbursed for treating it. They are also skeptical that by calling obese people “sick,” we are ensuring that they will be dependent on the healthcare system as the designation implies that the individual will need professional help.

But, does this announcement harm efforts to control the epidemic? My opinion is that it does not. I believe our position should be that the designation of obesity as a disease helps to remove the guilt and shame that obese people feel and allows physicians to counsel these patients without finger-wagging or lecturing.

We saw a similar resistance to calling addiction a disease. We feared that it would be too easy for the addict to abandon his personal responsibility toward his condition if he had a disease to blame. However, physicians have become increasingly convinced by treatment successes with pharmacotherapy, psychotherapy, group therapy, and research that addiction is a disease.
There are already existing theories that obesity is caused by food addiction (addiction to types of food, large portions, and impulsive eating). So, I am having trouble understanding why calling obesity a disease would trigger such an outcry from members of the medical community.
Perhaps they need to “lighten up”.

Breast Augmentation Taxation?

Breast augmentation taxation is on the platform of healthcare issues in 2009. The goal of the taxation for plastic surgery is to assist in providing healthcare coverage to people that cannot afford coverage on their own. Using breast augmentation as an example, let’s take a closer look at what the outcome of taxation could yield. Knowing that breast augmentation is the most popular plastic surgery procedure and may range in cost from $4,000.00 to $10,000, let’s consider the number of women that have had breast augmentation and the yield in dollars had taxes been levied on the procedure. According to the American Society of Plastic Surgeons, over 800,000 women in the U.S. selected the breast augmentation procedure in the past three years. If only 25% of the women that had breast augmentation purchased the procedure at the rate of $4000, the yield would bring millions of tax dollars available to help fund the uninsured. And the numbers only reflect an estimated 25% of breast augmentation procedures performed in the past three years. Consider the numbers when we account for all breast augmentation procedures or, for that matter, all plastic surgery procedures. The impact could be startling. But, why would we want to support such taxation?

Some may feel that taxation on plastic surgery procedures such as breast augmentation is a measure of good will. Others may justify the taxation because the procedures like breast augmentation use breast implants or other technology, and therefore should be taxed. Still others may be against the taxation. They may contend that taxation could significantly harm a highly successful industry that sets a bar for high quality care. From the comforting ambiance to the time the surgeon spends in consultation and in surgical care, plastic surgeons are able to stand above many other types of medical specialists because their services are not governed by external forces. We will not know for some time what the verdict will be. Though, we still know that plastic surgeons stand behind quality services. Even if taxation does come to fruition, the enhancement is life through plastic surgery is truly fulfilling. Breast augmentation will certainly continue to be a popular plastic surgery procedure.

2In a tummy tuck surgery, superfluous fat and skin in the lower and middle areas of the abdomen are removed. Through the tightening of the abdominal wall’s fascia and muscle, a flatter and firmer abdomen can be achieved. Tummy tuck procedures are of different kinds: traditional tummy tuck, mini tummy tuck, endoscopic tummy tuck, and extended tummy tuck. The procedure to be used for a particular patient would depend on individual requirements. A patient taking medicines may have to stop or decrease the dosage two weeks before the surgery. If a smoker, he/she would have to quit smoking 14 days ahead of the procedure.
